Partial Care

Pat LeBon Day Program/Empowerment Program serves adult consumers with a dual diagnosis. The program is designed to promote adult behaviors and attitudes by incorporating incentives to encourage constructive use of personal time. The program is organized around the normally recurring events of daily living. These events present many opportunities for training perceptual abilities, gross and fine motor skills, coordination, communication skills and increased independence in hygiene and self-care skills. A consulting psychiatrist actively monitors consumers’ medication needs and receives consistent input from staff. Consideration for the consumers’ references, based on assessment and experience, are integrated into all activities.

The Challenge Grant provides information on nutrition, menu planning, shopping, banking and budgeting. A Program Coordinator works with an individual an average of twelve hours per week or as needed. The individual receives transportation support and guidance for community involvement.

Group Homes

A group home affords individuals the opportunity to live as their abilities permit. When possible, staff will provide supports needed to help individuals become contributing participants in their neighborhood and community organizations. Each individual learns how to utilize local resources including banks, churches, grocery stores and recreational facilities. It is the goal of the group home to help our consumers meet the principles of determination: freedom, authority, support and responsibility.

Residential Services

Bayberry Apartments & Britton Place are programs designed to enable capable adult men and women achieve independence by living in the community and utilizing community resources. Consumers considered for this option are usually eligible for competitive employment or individual job placement. Staff is available 24 hours a day, 7 days a week, in the event of an emergency.

New Beginnings emergency capacity program provides highly structured supervised residence to up to four individuals during emergencies. The individuals live in an apartment setting. Services are provided to adults with dual diagnosis and are designed to be a temporary setting for up to 60 days.

SCI Program provides independent living for up to four young adults with developmental disabilities and are ready to live in the community with limited support.
