Adult Community Services

List of Services

Adult Outpatient Program offers individualized contact with a psychiatrist and a registered nurse. Individuals come at intervals for the monitoring of psychiatric medication only. As this is the least intensive mental health service offered, consumers who are enrolled must show that no other mental health services are required.

Seasons Counseling provides outpatient preventative and therapeutic mental health services, which promote well-being, community living and independent functioning. Psychiatric evaluation and medication monitoring are also provided in conjunction with therapy when clinically indicated. Consultation and education include assessment and referral for drug and/or alcohol, stress management, and substance abuse.

R.I.T.E. Center (Realizing Independence Through Empowerment) is a consumer run and managed program. The center is a self-help, drop-in center for all consumers of mental health services in the county. The R.I.T.E. Center provides a place for consumers to gather for socialization, emotional support, self-help, acceptance and education. Services and activities include providing information and referral, weekly self-help groups, recreational activities, help with daily living skills and linkage to additional services in the community.

I.C.M.S. (Integrated Case Management Services) provides intensive outreach case management services to adults with severe and persistent mental illness. The goals of the service are to reduce both the frequency and length of hospitalization, and to increase connections to community services that improve quality of life.

Supportive Housing

Haven provides independent living for up to ten adults suffering from severe and persistent mental illness, and are “aging out” of the children’s services system who otherwise would become homeless. Treatment includes case management and life skills training to assist with independent living.

Special Needs Housing Trust Fund provides capital financing to create
permanent supportive housing and community residences for individuals with special needs. The purpose of this program is to develop special needs housing and residential opportunities as alternatives to institutionalization or homelessness.

Partial Care Programs

Partial Care Programs provide consumers with the support needed to reduce or prevent psychiatric hospitalization and to improve functioning in the community. Staff assist individuals to move to a higher level both at home and in the community. Services include group and supportive individual counseling, case management, daily living skills, psychiatric support, medication monitoring, social skill building and pre-vocational training, specialized psycho-educational groups for unique populations. Residential Services

Medford Meadows serves adults with serious and persistent mental illness, age 45 or older, who have had a stay of at least six months at a State Psychiatric Hospital. The goal of this program is to support and encourage the development of life skills required to sustain successful community living. We emphasize the importance of consumer responsibility and participation, focusing on rehabilitation designed to develop and improve skills necessary for successful community integration.

Oasis provides community-based living for severe and persistent mentally ill adults who may also be developmentally disabled and have been hospitalized for minimally six months at Ancora State Psychiatric Hospital and are ready for discharge. The Oasis program’s therapeutic and nurturing living environment offers a safe and least restrictive setting which provides limits and structure to the consumers it serves. The program focuses on supporting and encouraging the development of life skills required to sustain successful living within the community. The program offers both partial care and residential care rehabilitation designed to develop and improve the skills necessary for successful community integration.
